Do I need a referral from my family doctor?

No, you can fill out the online referral form yourself and identify in the list provided or under others
your reason for contacting us.

No, you don’t have to have a diagnosis to arrange for a meeting with a psychotherapist.

Absolutely, your privacy is protected and we are not sharing your information with third parties unless
instructed by you.

Yes, if you kindly let us know your preference, we will see when this clinician becomes available. If you like to be seen sooner, we might refer you to the next available therapist.

Please fill out the online referral form or contact the clinic by calling 647.349.6886 and our clinic
manager will arrange for a first 15 min free of charge consultation with the next available therapist. We
will then email you our onboarding forms (Consent for treatment and Info sheet) to add you to our
system. An appointment can only be booked, once all these forms have been provided.

Absolutely, we do have a Multi Disciplinary Consent Form, where you can add any provider of your choice to the list identified as your team, so we can contact them.

A multi disciplinary team approach allows us as professionals to work with the same person and our
shared professional lens. This allows for more knowledge and understanding of the client’s situation.

No, OHIP does not cover our services. Please check with your Insurance or benefit provider, if the service of your choice is covered and how much you are eligible for. You will have to arrange for reimbursement directly with your provider.

Once your onboarding forms have been added to our system, you will be requested to provide us with your Credit Card for payment. Some people may have some coverage with their own insurance.

After each psychotherapy session or psychological service, our clinic will provide you with an Invoice.
